A Must for Your Students As They Begin the Human Disease Project - As your students begin this project, they will most likely do some research using the Internet. Have them fill out this form.
Interview Form - Hand out this form to guide students in setting up and conducting an interview.
Writing a Bibliography - Hand out this form to guide students in writing a bibliography.
Citing Internet Resources - If you get information from any Internet source, make sure you cite your references correctly.
Son of Citation Machine This website is great. Use the citation generator. It will create the bibliographical reference for your students that they can then copy and paste into their bibilography. This is easy!
HDP Student Self Assessment Form - Students can use this form to assess their performance.
Student Report Grading Form - Teachers, use this form to assess student reports. Won't necessarily work for "out of the box" reports.
California State and Federal Standards for Human Disease - Check the California State Standards for teaching about disease. I also reference them against the Federal Standards.
